About the Role:
The Substance Misuse Case Manager will be responsible for providing vital support to service users within the OAT team. Key responsibilities include:
- Conducting thorough assessments to evaluate the eligibility, strengths, and needs of service users, and developing tailored care plans.
- Continuously monitoring and reviewing recovery care plans to ensure timely and appropriate support for service users.
- Delivering individual and group psychosocial interventions aimed at increasing engagement and improving recovery outcomes.
- Conducting risk assessments and creating risk management plans in line with organisational policies.
- Collaborating with other healthcare professionals to ensure comprehensive care delivery.
- Acting as a key member of a multi-disciplinary team, contributing to team meetings and care planning discussions.
- Providing case management within a harm reduction and recovery-focused model, including one-to-one and group work interventions.
- Communicating sensitive information in a professional manner, managing difficult and emotive conversations with service users, families, and colleagues.
- Contributing to safeguarding decisions and collaborating with relevant agencies to protect vulnerable individuals.
- Compiling and presenting reports, participating in case conferences, and engaging in multi-disciplinary team meetings to ensure holistic care planning.
- Screening for alcohol use and collating statistical data as required for reporting purposes.
What We’re Looking For:
- Recent experience in Substance Misuse, Addiction, Drug and Alcohol Services, Recovery, Needle Exchange, or similar.
- Relevant qualifications or certifications in Substance Misuse or a related field.
- Strong communication skills, with the ability to engage effectively with service users in challenging situations.
- Ability to work independently and as part of a dynamic multi-disciplinary team.
