What You’ll Be Doing:
- Engaging Directly: Meet individuals where they are, in various community settings, and guide them towards available treatment and support services.
- Tailored Support: Conduct assessments to fully understand each person’s strengths and needs, and create bespoke care packages that address these holistically.
- Collaborative Approach: Work hand-in-hand with healthcare professionals as part of a multi-disciplinary team to deliver cohesive, well-rounded support.
- Empowering Change: Offer one-on-one and group sessions that focus on psychosocial support and therapeutic interventions, helping individuals build resilience and confidence.
- Advocacy & Outreach: Train internal and external stakeholders on key topics, and actively promote harm reduction and community resource access.
- Safeguarding Practices: Ensure safety and dignity in all interactions, accurately record data, and contribute to a safe, supportive environment.
What We’re Looking For:
- Qualifications: Degree or diploma in Social Work, Psychology, Substance Misuse, or a related field.
- Relevant Experience: Demonstrated experience in outreach or similar roles, ideally within the substance misuse or mental health field.
- Skills: Strong communication, organisational, and problem-solving skills; an empathetic, non-judgmental approach; and the ability to work both independently and collaboratively.
