The Role:

As a Case Manager, you will play a pivotal role within our community teams, providing essential long-term case management to adults facing substance misuse challenges. With a focus on personalised care plans and key working strategies, you will collaborate closely with our prescribing team to deliver specialist interventions and harm reduction measures. Your dedication will empower individuals to lead fulfilling lives within their communities.

Requirements:

  • Relevant experience in the substance misuse field
  • A degree in psychology, sociology, social work, or Diploma in Health and Social Care, or equivalent qualifications
  • For experienced candidates without formal qualifications, we offer comprehensive accredited training and support for the ASYE programme for newly qualified Social Workers
