Social Worker within Adults with Disabilities Team - Civic Centre, Wolverhampton

What you will do:
- Conduct Care Act Conversations using the 3 Conversation model.
- Work with adults with diverse needs including learning disabilities, complex care needs, dementia, and older adults.
- Undertake safeguarding enquiries and participate in Mental Capacity conversations.
- Engage in court work as necessary to support the best interests of the service users.
- Rotate on the duty rota serving adults within the East locality area of Wolverhampton.
Candidates must have experience in Learning Disabilities and Physical Disabilities and possess some knowledge of using Eclipse software.
