What you will do:

- Monitor service users’ progress within the reablement service and assess any ongoing needs.
- Manage a short-term caseload and review your priorities regularly.
- Conduct assessments under the Care Act 2014 and formal mental capacity assessments when needed.
- Address both statutory and non-statutory safeguarding concerns with thorough attention.
- Offer information, advice, and signposting to other agencies and services.
- Work closely within a small, supportive social work team, offering guidance to colleagues and apprentices.
- Participate in team meetings and relevant training opportunities.
- Network with other professionals and engage in multi-agency collaborations.
- Contribute your Social Work expertise to the wider Multi-Disciplinary Team, positively impacting the REaCH service.
