Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for an Adult Social Worker within the Rapid Response & Reablement Team to work Full Time based in Guernsey.

The salary for this Adult Social Worker job is up to £73,085 per annum.

Main duties:

  • To provide a social work service to adults living in the Bailiwick of Guernsey, with a focus on those on the Rapid Response and Reablement caseload. To carry out assessment using the agreed standardised format. To participate in co-ordinating services for people wishing to remain within their own homes. To offer support to people who may be moving into care.
  • To work in a flexible way, adapting the service to meet the changing needs of service users from a wide range of social and cultural backgrounds.
  • To contribute towards a holistic assessment within the multi-professional specialist team, recognising that diversity, values and human rights may influence the decisions of people who use the service.
  • To respect an individuals independence and dignity and to offer person-centred support in making major life decisions and choices.
  • Assess family’s emotional resilience, coping skills, strengths, and difficulties within the practise setting, using expert social skills & discussion with the multi-professional team.
  • Undertake person-centred assessments, evaluating & reviewing service users and family’s needs regularly and readjust care plans if necessary.
  • Take lead responsibility for providing consultation to the specialist team regarding adults at risk, and make referrals as necessary regarding child protection & adult at risk.
  • Take special responsibility for recognising and addressing the needs of those with mental health problems, learning disabilities, and other vulnerabilities.
  • Manage an individual caseload, organising and prioritise the workload effectively, including timely discharge of patients from the caseload.
  • Attend and facilitate meetings for the core and wider multi-disciplinary team to discuss complex cases, practise issues and innovative work.
  • To demonstrate and model a high standard of practice under their own direction, that reflects a value based approach to working with adults and families that is person-centred.
  • To be the Lead Professional in the team for adult safeguarding in the identified service area. This will include mentoring and supporting others.
  • To maintain up to date knowledge of research to practice and contribute to its implementation.

Requirements of this Social Worker job:

  • A social Work Qualification or equivalent.
  • Understanding of relevant legislation.
  • Registered or eligible for registration.
