Job Description:
We are currently seeking a passionate and dedicated Adult Social Worker to join our Locality Team. As an Adult Social Worker, you will be responsible for assessing the needs of adults requiring social care support, developing care plans, and coordinating services to promote independence and improve quality of life. You will work collaboratively with service users, their families, and other professionals to deliver person-centred care and support.
Key Responsibilities:
- Conducting assessments of individuals' care and support needs.
- Developing and implementing care plans in collaboration with service users and their support networks.
- Liaising with external agencies and professionals to coordinate services and support.
- Providing advice, guidance, and advocacy to individuals and their families.
- Monitoring and reviewing care plans to ensure they remain effective and responsive to changing needs.
- Participating in multi-disciplinary team meetings and case conferences.
Requirements:
- A recognised qualification in Social Work (e.g., Degree in Social Work, DipSW, CQSW).
- Registration with Social Work England.
- Experience working within Adult Social Care, preferably within a locality or community setting.
- Strong assessment and communication skills.
- Ability to work effectively within a multi-disciplinary team.
- Commitment to promoting independence, dignity, and empowerment for service users.
