What you will do:
- Engage with adults within the community to assess their needs and develop tailored care plans.
- Work collaboratively with other professionals within the Neighbourhood Team to ensure cohesive and effective support for clients.
- Conduct regular reviews and evaluations of individual progress and adjust care plans as necessary.
- Advocate on behalf of clients, ensuring they have access to necessary resources and services.
- Maintain accurate and comprehensive records according to statutory requirements.
Requirements:
- Possess a minimum of three years’ experience in social work ideally with exposure to neighbourhood work.
- A full driving licence is preferred to facilitate client visits.
- Capability to work in the office 2 - 3 days per week is required to align with team collaboration and meetings.
