What you will do:

- Provide safe and effective care and support for adults at risk, ensuring their basic needs, as detailed in support plans and risk assessments, are fully met.
- Deliver person-centred quality care tailored to the unique needs and aspirations of each individual, prioritising dignity, respect, and compassion.
- Aid in the delivery of agreed care plans, ensuring quality, safety, and alignment with service targets and CQC standards.
- Assist clients with personal care tasks such as dressing, bathing, toileting, and medication administration according to individual plans.
- Monitor service users' progress and be vigilant about behavioural triggers, updating managers as necessary.
- Support emotionally demanding individuals and alert managers to significant issues.
- Collaborate with senior officers to implement motivating activities and contribute to the planning of daily activities and risk assessments.
- Participate in and contribute positively to team meetings, sharing valuable insights and updates regarding service users.
- Accurately administer and record medication following the Council's Medication policy.
- Maintain updated records, ensuring compliance with information security protocols.

Essential requirements:

- Experience as a Social Worker in Adult Mental Health and MASH.
- Capability to provide physical assistance for personal care tasks.
- Strong written communication skills for report writing using standard templates.
- Commitment to continuous learning and development.
- Ability to work as part of a team and communicate effectively.
