Sanctuary Personnel are seeking a dedicated professional to join our client's adult disability service, supporting individuals with learning disabilities across various settings including residential homes, supported living, short break services, and outreach teams. The role requires working within a shift system providing 24-hour support.

You will engage in all aspects of maintaining the service area using the active support model, encouraging service users to participate in activities such as cleaning, shopping, washing, ironing, and cooking. A valid driving licence is required, although some flexibility may be offered.

As a Band 6 professional, you will coordinate and lead a team, and develop and implement care and support programs.

Summary:

  • The individual in this role is a part of the adult disability service, which assists individuals with learning disabilities. The position can be situated in various environments, including residential homes, supported living facilities, short break services, and the outreach team.
  • Staff members are required to work a shift system that provides support 24 hours a day. They are involved in all aspects of maintaining the environment using the active support model, which may include tasks such as cleaning, shopping, laundry, ironing, and cooking. Service users are encouraged to participate in these activities alongside the staff.
  • Staff are expected to hold a driving license and be prepared to drive service vehicles, although the manager may allow some flexibility with this requirement.
  • Band 6 staff members are expected to coordinate and lead a team within the service and develop and implement care and support programs.

Main duties:

  • Conduct assessments for service users with complex needs.
  • Develop, implement, and evaluate care programs tailored to meet these needs.
  • Guide, advise, and direct other staff in the assessment, planning, implementation, and evaluation of the care provided to service users.
  • Carry out risk assessments to identify risks to the health and well-being of service users with complex needs, and take appropriate action to minimize these risks.
  • Lead a team of staff in emergency situations, ensuring the service user’s needs are met and providing support to significant others.
  • Use information and data from various sources to identify service users' needs and inform decision-making processes.
  • Act in accordance with the standards, codes, and guidelines established by the post-holder’s regulatory body and HSC.
  • Promote service user involvement in the planning, delivery, and evaluation of care, respecting their wishes, beliefs, and dignity.
  • Safeguard service users in accordance with organizational policies and guidelines.

Requirements of the role:

Qualifications:

  • Registered Nurse or Social Worker.
  • Evidence of CPD (Continuing Professional Development) at diploma level.
  • Teaching/mentoring qualification.
  • Diploma in a related field.
  • Level 3 Certificate in Assessing Vocational Achievement or equivalent (D32/33, NVQ A1).

Essential Skills:

  • Good IT skills.
  • Excellent verbal and written communication.
  • Valid driving licence (negotiable with the manager depending on the area).
  • Leadership and management skills.
  • Caring and empathetic nature.
  • Understanding of learning disabilities and/or willingness to learn.
