Clinical Audit Lead – Continuing Care Assessments

About the Role
As a Clinical Audit Lead, you will work collaboratively with a multidisciplinary team, including Social Workers, Support Workers, and Occupational Therapists, to support and assess Continuing Care applications. You will provide expert advice, ensure compliance with standards, and contribute to service improvement to deliver effective, person-centered care plans.

Key Responsibilities

  • Provide expert clinical advice to stakeholders on continuing care cases.
  • Support Social Workers in completing Continuing Care Checklists, ensuring high standards are met.
  • Communicate complex, sensitive case information to ensure appropriate care planning.
  • Participate in panels such as Multi-Agency Resource Forums, offering clinical guidance and signposting to additional health services where needed.
  • Conduct audits of care plans to evaluate service effectiveness and recommend improvements.
  • Work collaboratively with health and social care partners to maintain compliance with legislative frameworks.
  • Stay updated on legislative changes and Department of Health guidance to ensure policies remain relevant.

About You
To excel in this role, you will need:

  • A qualification as an RGN/RMN and current NMC registration, with evidence of CPD.
  • Experience or knowledge of assessing NHS Continuing Healthcare (CHC) and Funded Nursing Care (FNC).
  • A track record of collaborating with multiple stakeholders across public and private health sectors.
  • Strong communication and facilitation skills, with the ability to manage complex information.
  • A full UK driving license or alternative mobility arrangements to meet role requirements.
