Shift Pattern: Full time. Shift work (long days, nights, late, sleep over & early shifts).

About the Role

As a Registered Nurse specialising in Learning Disability, you will play a pivotal role in supporting the health and well-being of individuals with PMLD residing in their residential nursing home. You will lead and manage shifts, ensuring the highest standards of care are upheld while empowering individuals to live fulfilling lives.

Job Purpose

This role spans across various clinical areas within both acute and community services. You will be responsible for planning and delivering an effective and competent level of care, being fully aware of patients' needs, and upholding the highest standards in patient care.

Your key responsibilities will include:

  • Providing personalised care and support tailored to the unique needs of each individual with PMLD.
  • Administering medication and monitoring health conditions.
  • Collaborating with family members and healthcare professionals to ensure holistic care.
  • Leading and supervising a team of care staff, ensuring compassionate and efficient care delivery.
  • Developing and implementing care plans and risk assessments, regularly reviewing and updating them to reflect individuals' changing needs.
  • Fostering a supportive and inclusive environment that promotes independence, dignity, and quality of life.
  • Adhering to regulatory standards as set by the Jersey Care Commission.

Other Responsibilities:

  • Collaborating with colleagues across departments to ensure continuity of care, acting as a role model for patients, relatives, and colleagues.
  • Supervising the care provided by Healthcare Assistants and offering guidance and training where appropriate.
  • Demonstrating a thorough knowledge of the NMC Code (2015) and applying it to maintain trust and credibility.
  • Keeping up to date with developments in clinical and nursing practice, sharing knowledge and expertise with the team.
  • Contributing to the development of a therapeutic environment and setting high standards in the clinical area.
  • Participating in clinical audits, enhancing nursing care through evidence-based practice.
  • Demonstrating effective infection prevention measures and challenging poor practice.
  • Providing high-quality advice to patients, carers, and colleagues, promoting health and well-being.
  • Taking responsibility for the safeguarding and welfare of all individuals within your care, adhering to all relevant policies and procedures.
  • Maintaining accurate patient documentation and producing high-quality patient information.
  • Leading and directing the team when required, working collaboratively as part of a multidisciplinary team.
  • Engaging in continuous professional development, participating in performance reviews, and assisting junior staff in meeting their learning objectives.

Person Specification

Essential Qualifications:

  • 1st Level Registered Nurse with NMC Registration.
  • Minimum of 2 years post-qualification experience working across Learning Disability Nursing.
  • Thorough knowledge of the NMC Code (2015).

Desirable Qualifications:

  • Commitment to work towards a Level 2 vocational award in Health and Social Care.
  • Teaching and Mentorship qualification.

Skills and Experience:

  • Excellent communication and team-working skills.
  • Basic keyboard skills.
  • Ability to manage risks and complaints.
