What you will do:
- Assist qualified occupational therapists and physiotherapists during community visits.
- Promote patient independence and reablement by supporting therapy plans.
- Collaborate with team members to develop effective care strategies.
- Conduct assessments and provide regular feedback to the supervising therapist.
- Help devise personalised rehabilitation plans for clients.
- Maintain accurate and timely patient records.
Must be a car driver. Daily journeys to patients’ homes in the Sefton area are essential for this role.
