What you will do: Undertake assessment and care management for clients, promoting a multidisciplinary approach. Manage an allocated caseload, prioritising and organising your workload. Assess for and facilitate provision of equipment, following best practices and Council policies. Plan and facilitate minor and major adaptation work, collaborating with other agencies. Identify resources to meet the needs of service users. Develop care plans in consultation with children and their carers. Follow national and local procedures regarding data protocols and communication strategies. Promote community rehabilitation for service users.
