Duties:
- Completing a comprehensive geriatric assessment of the new admissions and following up new admissions from the out of hours period
- ‘Clinical clerking’ of patients (formerly medical clerking) comprises of clinically assessing patients, developing, evaluating and implementing programmes of care using specialist, analytical and judgemental skills to formulate a plan of care
- Reviewing patient’s medical care; diagnosis, planning and arranging diagnostics, in consideration of frailty, prognosis and patients wishes
- Medications reviews, de-prescribing of necessary, working closely with the ward pharmacy team
- Reviewing acutely deteriorating patients (eg sepsis, chest pain, stroke) on the ward in a timely fashion and formulating and initiating an appropriate management plan
- Leading the team in an emergency response to a deteriorating patient on the ward
- A daily (7.5 hour) presence to support care of patients, able to provide a timely response to the clinically deteriorating patient and provide guidance and education relating to any uncertain situations as they arise
- Providing all aspects of end of life care; important conversations with patients and families/ carers leading to advance care planning; DNACPR / ReSPECT, preferred place of death, prescribing end of life medications
- Making the decision to discharge and with the MDT formulate the EDD (expected date of discharge) on admission
- Support the ward team in achieving patient goals and empowering nursing staff to do this
- Support the use of intra venous therapy on the ward
Requirements for the Emergency Nurse Practitioner role:
- Advanced Clinical Practitioners/Advanced Nurse Practitioners (need advanced physical assessment skills and a non-medical prescribing qualification)
- Experience of working with the frail, older population and end of life care
- Competent in assessing and managing the acutely unwell patient
