Community Advanced Nurse Practitioner (ANP) – Community Rapid Response Service

Sanctuary Personnel is recruiting an experienced and skilled Advanced Nurse Practitioner (ANP) to join our client’s dynamic Community Rapid Response Service in Guernsey. This multidisciplinary service plays a vital role in crisis intervention, aiming to prevent unnecessary hospital or care home admissions. Operating seven days a week, the service provides essential short-term support to help patients maintain their well-being and stay in their homes during periods of crisis.

As an ANP, you will be responsible for delivering patient-centred care, including assessment, diagnosis, treatment, and follow-up care within the community. You will work collaboratively with GPs, emergency services, and other healthcare professionals to manage complex cases, ensure smooth transitions in patient care, and maintain the highest quality of care.

Key Responsibilities:

  • Leadership in Patient Care: Lead patient assessments, care planning, and delivery, ensuring positive outcomes.
  • Advanced Clinical Practice: Conduct comprehensive health assessments, manage complex cases, and deliver evidence-based interventions.
  • Autonomous Decision-Making: Make independent clinical decisions, collaborating with medical professionals when needed.
  • Multidisciplinary Collaboration: Work with other healthcare providers to manage patient referrals, discharges, and ongoing care.
  • Patient Involvement: Actively involve patients and their families in care planning and decision-making.

Educational & Developmental Responsibilities:

  • Health Promotion: Lead health and well-being initiatives for patients and families.
  • Staff Development: Support junior staff development through training, supervision, and ongoing learning.
  • Continuous Learning: Commit to professional development to stay at the forefront of advanced clinical practice.

Key Criteria:

Essential:

  • Registered Nurse with at least 5 years of relevant community experience.
  • Master’s degree in Advanced Practice (ANP) or equivalent.
  • Non-medical prescribing qualification.
  • Management qualification.
  • Proven leadership and excellent communication skills.
  • Strong interpersonal and organisational abilities.

Desirable:

  • MSc in Management or advanced clinical skills qualification.
