About the Role

Sanctuary Personnel are seeking a Specialist Rehabilitation Practitioner to join our client, a leading care and rehabilitation centre based in Surrey. This state-of-the-art facility offers a range of client pathways, including Neuro Rehabilitation, General Rehabilitation, Complex Neurological Care, and Outpatient/Respite Services.

As an autonomous practitioner, you will collaborate closely with acute services, GPs, and other referring professionals to assess clients for their inpatient rehabilitation services. Your role will involve clinical triage, client assessments, and determining the most suitable care pathway based on specialist clinical reasoning, evidence-based practice, and enhanced decision-making skills.

This position is pivotal in developing a trusted assessor process for referrals into the Care and Rehab Service.

Key Responsibilities

  • Professionally and legally accountable for your own work and that of your team.
  • Conduct specialist holistic rehabilitation assessments, including complex cases, using advanced clinical reasoning skills.
  • Manage triage calls effectively, employing skilled questioning, active listening, and excellent verbal communication.
  • Evaluate patient progress, write detailed reports, and ensure effective handovers.
  • Communicate tactfully with patients, family members, and colleagues, especially in cases where there are communication challenges.
  • Adhere to all relevant policies and procedures.

Candidate Requirements

  • Professional Qualification: You must hold a relevant healthcare qualification (e.g., Occupational Therapist, Physiotherapist, Nurse) with current registration to practice in the UK.
  • Experience: A minimum of 3 years of post-qualification experience in a rehabilitation setting, particularly with neurological conditions, is essential.
  • Clinical Expertise: Strong clinical reasoning, assessment, and problem-solving skills are crucial, particularly in managing complex cases with multi-pathologies.
  • Communication Skills: Excellent verbal and written communication skills, with the ability to engage effectively with patients, families, and healthcare professionals.
  • Team Player: A collaborative approach and the ability to work as part of a multidisciplinary team are essential.
  • Flexibility: Willingness to adapt to the varying needs of the service and to participate in continuous professional development.
