We are looking for a committed and hardworking qualified Social Worker with Adult Social Work experience working with older people, individuals with physical disabilities and sensory impairments, learning disabilities, as well as mental health issues. You need to be motivated and flexible, maintaining high professional standards and possessing a sound understanding of current legislation including the Care Act 2014, Safeguarding, Continuing Health Care, Mental Health Act, Mental Capacity Act 2005, and Community DOLS/LPS.
Primarily, the role involves tackling waiting and review lists. You will engage with vulnerable adults and their carers, applying an asset-based and strength-based approach to help them achieve their goals within the wider community. Confidence is key in undertaking safeguarding investigations, mental capacity assessments, best interest decisions, carrying out outcome-focused assessments, and support planning to meet care and support needs.
You must excel in working with a diverse group of individuals and partner agencies to deliver the best outcomes for customers and their carers. A degree in Social Work, Dip SW, CQSW or equivalent is essential for this role.
What you will do:
- Support with the waiting list and review list for adults with varying needs.
- Work with vulnerable adults and their carers with an asset-based and strength-based approach.
- Undertake safeguarding investigations and mental capacity assessments.
- Carry out best interest decisions, outcome-focused assessments, and review assessments with support planning.
- Collaborate with partner agencies to deliver optimal outcomes for customers and their carers.
