What you will do:
- Conduct assessments to identify risks for vulnerable adults and devise strategies to mitigate them.
- Develop and implement care plans tailored to individual needs, working collaboratively with residents and their support networks.
- Carry out home visits and telephone assessments to monitor residents' safety and well-being.
- Coordinate with external agencies, healthcare professionals, and community resources to deliver comprehensive support to residents.
- Participate in multidisciplinary meetings to review resident progress and develop intervention strategies.
- Provide crisis intervention and immediate support to residents experiencing safety concerns.
- Maintain accurate and timely records of resident interactions, assessments, and interventions in line with regulatory standards.
A successful candidate will have a degree or equivalent in social work, current registration with Social Work England, and at least three years of significant experience in adult social care and crisis intervention.
