What you will do:

  • Provide tailored social work services to vulnerable adults with learning disabilities, ensuring their rights, dignity, and self-determination remain central.
  • Conduct thorough investigations and assessments, crafting detailed, evidence-based reports for highly complex cases.
  • Develop and implement personalised support plans, coordinating necessary services while navigating financial constraints and authorising care packages.
  • Monitor and review support plans, ensuring consistent delivery and adapting to changing needs as required.
  • Perform safeguarding investigations and manage risks effectively to protect individuals in your care.
  • Facilitate joint assessments with partner organisations to foster a multi-disciplinary approach to care.
  • Offer detailed information and guidance to clients and caregivers regarding available local services and supports.
  • Maintain up-to-date and accurate records within the department’s IT systems as a means for effective service delivery.
  • Lead case work, providing ongoing advice and supporting vulnerable clients and their carers.
