What you will do:
- Engage in comprehensive social care needs assessments to determine appropriate interventions.
- Develop support plans tailored to the individual needs of service users.
- Commission services effectively, ensuring the best resources are available to those in need.
- Conduct risk assessments and manage risks, attending Multi-Agency Risk Assessment Meetings (MARAMS).
- Safeguard adults by upholding the principles of the Mental Capacity Act (MCA) and the Best Interests (BI) framework.
- Review and manage Court of Protection (COP) applications and Deprivation of Liberty Safeguards (DOLS).
- Liaise with healthcare professionals for Continuing Healthcare (CHC) assessments, working through the Decision Support Tool (DST).
- Collaboration within a Combined Localities Team to support a busy Adults Community/Locality team.
