About the Role
Sanctuary Personnel, an award-winning recruitment agency, is seeking a dedicated Senior Carer Team Leader to join an established Adult Community Care Service in Guernsey. Working closely with the Social Care Team Coordinator, this role is integral to delivering high-quality, person-centred care within the Home Care Service.
The Team Leader will oversee daily operations, supporting the assessment, planning, and implementation of care plans to help service users live safely and independently in their own homes. This role is essential in maintaining consistency and quality of care within the community.
Main Duties and Responsibilities
Care Planning & Delivery:
- Assess individuals referred for home care and design tailored care packages to meet their needs.
- Implement and evaluate care plans, ensuring they promote self-care and independence.
- Work directly with staff to deliver and maintain high standards of care.
Leadership & Team Support:
- Provide guidance and advice to staff in planning and delivering person-centred care.
- Assist in risk assessments for individuals with complex needs, taking appropriate actions.
- Support staff during emergencies, ensuring the needs of service users and their families are met.
Collaboration & Communication:
- Represent the Home Care Service at case management and care planning meetings.
- Encourage service user involvement in care plans, respecting their beliefs and dignity.
- Liaise effectively with multidisciplinary teams and external agencies to ensure seamless care delivery.
Safeguarding & Compliance:
- Follow safeguarding policies and escalate concerns appropriately.
- Use data and information effectively to inform decision-making.
What You’ll Need
Qualifications (one of the following):
- Teaching/mentoring qualification, diploma in a related field, or NVQ A1 assessor’s award.
- Registered health or social care professional with evidence of CPD at diploma level.
- Health and Social Care Degree.
- A commitment to professional development and strong leadership and communication skills.
