What you will do:

  • Act as a Personal Advisor to young people, ensuring the delivery and coordination of services in line with The Leaving Care Act 2000.
  • Maintain a caseload as a key worker for care leavers and co-work with Social Workers.
  • Provide advice and support to young people aged 16-21 (or up to 25 if in training or education) as they transition to adulthood.
  • Develop, implement, monitor, and review Pathway Plans for care leavers within statutory timescales.
  • Conduct proactive, outcome-focused direct work with young people both individually and in groups to build self-esteem and life skills.
  • Create comprehensive contingency plans for care leavers and support them in accessing community resources, health and leisure services, education, training, and employment.
  • Promote full involvement and participation of young people in service delivery, encouraging links within the community and maintaining family ties.
  • Collaborate with Social Workers and other professionals to access resources and services for young people.
  • Prepare reports and contribute to statutory Children in Care Reviews, court hearings, and multi-agency meetings.
  • Advocate for young people, guiding them to access independent advocacy and legal services, and ensuring they are informed about the complaints procedure.
