The salary for this permanent Rotational Clinical Pharmacist job is £43,742 - £50,056 per annum.

Main Duties:

  • Provide Specialist Clinical Pharmacy Services to a Specialty
    (Rotating to a different specialty every 6-12 months) in alignment with Pharmacy, Directorate, and Trust objectives:

    • Adhere to statutory requirements, the Controls Assurance framework, Department of Health guidance, GPhC Code of Ethics, Trust policies, and objectives.
    • Establish, maintain, and strengthen effective working relationships with Trust managers, professionals, departments, service users, and external organisations.
    • Supervise pharmacists, pre-registration pharmacists, and technicians working within the specialty, ensuring they meet competency standards.
    • Collaborate with your line manager to identify opportunities and risks, and to develop and enhance the quality of service within the specialty.
    • Keep your line manager informed about service issues, staffing, and other resource needs for delivering pharmaceutical care to patients.
    • Contribute to the Trust Pharmacy Service Strategy in consultation with the Senior Pharmacy Management Team.

    2. Provide Specialist Clinical Pharmacy Services to Designated Wards

    • Ensure a high standard of patient-focused care in accordance with the principles of Clinical Governance.
    • Ensure compliance with Medicines Management, Controls Assurance Policies, and Treatment Protocols.
    • Investigate patients’ drug histories, where appropriate.
    • Apply clinical pharmacy expertise to improve patient outcomes through the clinical validation of prescriptions (including discharge letters (eDD)) for legality, clarity, clinical accuracy, and suitability of medication for the patient (including assessing and utilising patients’ own drugs (POD)).
    • Monitor the clinical use of drugs in patients, including reporting near misses in prescribing and administration, as well as addressing risk management issues.
    • Coordinate the timely supply of medication to patients, in collaboration with nurses, medical staff, the ward Pharmacy Technician, or Dispensary staff.

    3. Additional Duties

    • Provide medicines information and advice to prescribers, nurses, midwives, and other healthcare professionals to optimise drug therapy.
    • Devise and monitor the storage of medicines on the wards to ensure potency, security, and quality are maintained.
    • Work closely with the nursing and medical teams to develop and enhance services within the specialty, supporting change management initiatives.
    • Offer advice to patients and carers to improve adherence to medication regimens and assess the need for compliance aids.
    • Prioritise your own workload and, if necessary, that of other Pharmacy staff to ensure patient and service needs are met.
    • Participate in a rotational training programme designed to provide specialist experience in Clinical Pharmacy and managerial duties.
    • Provide cover for other ward pharmacists as needed.

Requirements of the Rotational Clinical Pharmacist:

  • Pharmacy degree
  • Completion of 1-year pre-registration training
  • Registered member of the General Pharmaceutical Council (GPhC)
  • Dispensing and checking skills
  • Clinical pharmacy experience within a hospital setting
  • Experience in providing medicines information
  • Ability to travel between sites and for On-call duties
