Sanctuary Personnel, a dedicated and award-winning recruitment agency with a TrustPilot score of 4.9/5 and nearly 1000 reviews is currently looking for an experienced and dedicated Cross Sector Advanced Clinical Pharmacist to work in Lincoln on a full-time basis.
The salary for this permanent Cross Sector Advanced Clinical Pharmacist job is £50,952 - £57,349 per annum.
Main Duties:
- Deliver high standards of patient-focused care by adhering to Clinical Governance principles.
- Ensure compliance with Medicines Management, Controls Assurance Policies, and Treatment Protocols.
- Investigate and assess patients' drug histories, including evaluating patients' own medications (POD) for reuse.
- Review prescriptions for legality, clarity, clinical accuracy, and suitability for the patient.
- Monitor the clinical use of medications, reporting near misses and risk management issues related to prescribing and administration.
- Coordinate timely medication supply in collaboration with nurses, medical staff, ward Pharmacy Technicians, and Dispensary staff.
- Build and maintain strong working relationships with service users and other healthcare professionals.
- Provide advice and support to patients and carers to improve adherence to medication regimens, assessing the need for compliance aids.
- Prioritise workload to meet patient and service needs, managing the workload of other Pharmacy staff when necessary.
- Identify patients requiring additional clinical interventions due to complexity or high risk (e.g., polypharmacy).
- Enhance the discharge process by improving efficiency and safety in transferring patient care to primary care settings.
- Contribute to training and education programmes for Pharmacy, Nursing, Medical staff, and other healthcare professionals.
- Provide professional cover for the site Medicines Information Service as needed.
- Support the MDT in patient discharge processes, reviewing EDDs to minimise documentation errors and expedite discharges.
- Develop cross-sector services to identify patients at risk of admission or readmission, providing necessary interventions.
Clinical Pharmacist Duties in GP Practice
- Conduct medication reviews for patients with multiple or single medical issues (e.g., COPD, asthma), optimising treatment.
- Provide structured reviews by assessing ongoing medication needs and monitoring requirements.
- Support patients with medication queries and provide necessary guidance.
- Recommend medicines optimisation strategies to the MDT and attend multidisciplinary case conferences.
- Manage a caseload of patients with minor ailments within your competence.
- Signpost to other healthcare professionals or escalate cases to GPs as appropriate.
- Support the MDT and healthcare teams (e.g., community pharmacy) with medication-related queries.
- Audit and review medication use to identify and address issues linked to unplanned hospital admissions and readmissions.
- Reconcile medications post-hospital discharge and in transitions to care homes.
- Ensure continuity of medicine supply to high-risk patient groups.
- Analyse and present medicines data to inform decision-making and highlight risks.
- Undertake clinical audits of prescribing practices, providing feedback and implementing changes with the Practice team.
- Audit Practice compliance with national guidelines and CQC standards concerning medicines.
Supervision of Dispensing Activities
- Perform clinical checks of prescriptions to ensure legality, accuracy, appropriateness, and policy compliance.
- Prioritise and clinically check prescriptions in the correct order.
- Conduct final checks on dispensed medicines and extemporaneous preparations as needed.
- Provide medicines information and advice to patients, carers, and healthcare professionals.
- Dispense prescriptions, manage stock medicines, and handle Controlled Drugs as required.
- Ensure compliance with health and safety legislation governing the handling of medicines.
- Address complaints from wards, departments, or patients efficiently and compassionately, following Trust policy.
Support of Evidence-Based and Cost-Effective Practice
- Assist in the clinical evaluation of new pharmaceutical products for inclusion in the Trust Medicines Formulary, using unbiased sources.
- Participate in clinical audits and contribute to Research and Development initiatives.
- Conduct drug utilisation reviews and review Practice formularies to ensure optimal use of medicines.
Risk Management in Medicines Usage
- Participate in risk management audits and report medicines-related incidents using the appropriate systems (e.g., Trust Datix).
- Complete intervention reports to monitor the role of pharmacists in risk management.
- Ensure compliance with regulations such as COSHH, National Patient Safety Agency, and MHRA guidelines.
- Identify and report the use of unlicensed medicines or off-label use according to MHRA requirements.
- Participate in medicine recalls as necessary.
Requirements of the Cross Sector Advanced Clinical Pharmacist:
- MPharm degree
- Registered with the General Pharmaceutical Council (GPhC)
- Postgraduate Clinical Diploma or equivalent experience
- Independent Prescriber qualification
- Some experience in a supervisory role
- Extensive specialist medical knowledge acquired through training and experience
- Proven experience in medicines management within a hospital setting across various specialties
- Ability to travel between sites and for On-call duties
