Key Responsibilities:
- Patient-Focused Care: Support lead pharmacists in delivering a high-quality, patient-centred medicines optimisation service in line with nationally approved standards.
- Therapeutic Oversight: Monitor therapeutic programmes, offering expert advice on safe and economical medication use to optimise patient outcomes.
- Medication Management: Conduct medication reviews, medicines reconciliation, and pharmaceutical care planning to minimise risks during care transitions.
- Clinical Consultation: Collaborate with healthcare teams, participate in ward rounds, and provide complex clinical pharmacy expertise to peers, patients, and management.
- Policy and Best Practice: Assist the Clinical Pharmacy Manager in developing and implementing local guidelines, policies, and training to enhance clinical pharmacy services.
Requirements:
- Master’s Degree in Pharmacy (four-year vocational degree).
- General Pharmaceutical Council Registration, obtained via a one-year postgraduate pre-registration training programme with a final examination.
- Postgraduate Certification: Completion of a postgraduate certificate in Clinical Pharmacy, or the first year of a Diploma/MSc in Clinical Pharmacy, or the RPS postgraduate Foundation Programme.
