Occupational Therapist – Band 6 – Community

What you will do:
- Assess patients in their homes who have been referred for occupational therapy intervention, tailoring care plans to suit individual needs.
- Perform remote assessments and expertly triage visits as necessary to optimise patient care.
- Work collaboratively with a multidisciplinary team to ensure comprehensive support for patients.
- Maintain accurate records and documentation to facilitate seamless communication and care continuity.
In addition to a minimum of two years of experience, you will need to be HCPC registered. While a driving licence is preferable, it's not mandatory. You will have the chance to partake in hybrid working, perfectly blending remote and on-site practices.
