Key Responsibilities: Conduct home visits for patient assessments, therapy provision, and equipment provision, working independently or alongside a dedicated team. Collaborate with multidisciplinary teams (MDTs) to support urgent community responses and discharge processes. Provide comprehensive occupational therapy services, ensuring each patient receives personalised and attentive care tailored to their needs. Maintain accurate patient records, documenting progress and adjusting treatment plans as necessary to ensure optimal outcomes. Requirements: A full driving licence and access to a car is essential due to the community-based nature of the role. HCPC registration is required to practice as an Occupational Therapist in the UK. A minimum of two years’ relevant experience, particularly in community-based settings.
