What you will do:

  • Provide care to patients referred by Sutton GPs, conducting home visits, care home assessments, and community engagements as needed.
  • Assess and treat adult patients with neurological conditions, acquired communication, and swallow difficulties in the community.
  • Apply your experience in managing dysphagia, ensuring that you are dysphagia trained.
  • Engage in rehabilitation and long-term condition management, focusing on patients with conditions such as stroke, head injury, Parkinson's disease, multiple sclerosis, and other progressive conditions.
  • Collaborate effectively within a multidisciplinary team and carry out community visits throughout Sutton in Southwest London.
  • Ensure patients receive the best care by utilizing your EMIS training and NHS SMART card experience.


Requirements:

  • Must be registered with HCPC.
  • A minimum of two years' experience working with adult patients, particularly those with neurological conditions, is essential.
  • Being a car driver and having a vehicle for this role is highly preferable to cover the extensive community visits required.
