What you will do:
- Conduct comprehensive community assessments to support clients following hospital discharge or to prevent re-admission.
- Develop and implement personalised treatment plans to enhance the quality of life for clients within the community.
- Collaborate with other healthcare professionals and community services to ensure a holistic approach to client care.
- Maintain accurate records of client progress and treatment outcomes.
- Offer guidance and support to family members and caregivers to facilitate ongoing rehabilitation at home.
Requirements:
- HCPC registration required.
- Minimum of 2 years of experience working as an Occupational Therapist.
- Car driving is preferable to effectively reach clients across the region.
