What you will do:
- Conduct comprehensive assessments and treatments for patients within the community, fostering independence and improving their quality of life.
- Develop detailed care plans tailored to each individual's needs, ensuring effective and personalised care.
- Collaborate with a multidisciplinary team, including healthcare professionals, families, and carers, to achieve the best outcomes for your patients.
- Maintain accurate and up-to-date records of patient progress and interventions, adhering to professional standards.
- Use your own car to visit patients, providing hands-on support and interventions across Swansea.
Requirements of the Occcupational Therapist:
- Degree or equivalent in Occupational Therapy
- Recent UK experience working as an Occupational Therapist
- Recent NHS experience
- Registered with the HCPC
