Role Overview:

Sanctuary Personnel is seeking a highly skilled Advanced Nurse Practitioner (ANP) to join our client's dynamic Community Rapid Response Service. This multidisciplinary service focuses on crisis intervention, aiming to prevent unnecessary emergency admissions to hospitals or care homes. Operating seven days a week, the service is integral to maintaining patient well-being within their own homes during periods of crisis, offering short-term support where needed.

As an ANP, you will deliver patient-centred clinical care, including assessment, examination, diagnosis, and treatment within the community setting. You will collaborate closely with other healthcare professionals, including GPs and emergency services, to ensure safe referrals, discharge, and the ongoing care of patients with complex needs. Your advanced clinical skills and autonomous practice will be essential in supporting adult community services and delivering high-quality care.

Key Responsibilities:

  • Provide leadership and support in assessing, planning, and delivering patient care within your areas of responsibility.
  • Monitor practice standards, addressing any factors that may impact patient care quality.
  • Use data from various sources to guide decision-making and develop strategic directions for service delivery.
  • Promote service user involvement in the planning, delivery, and evaluation of care.
  • Ensure adherence to professional codes of conduct and standards within your sphere of responsibility.
  • Maintain effective communication channels between your practice area and other relevant departments and agencies.

Advanced Clinical Responsibilities:

  • Facilitate tailored care packages within your competence and guidelines, acting as a key member of a multidisciplinary team.
  • Provide comprehensive health assessments based on advanced knowledge and skills.
  • Contribute directly to the assessment, treatment planning, and care evaluation of patients in their home environment.
  • Prescribe care elements and pharmacological interventions as necessary, and order and interpret diagnostic tests.
  • Make autonomous clinical decisions, with support from medical professionals as needed.

Educational & Developmental Responsibilities:

  • Lead the promotion of health and well-being for patients and their families.
  • Support the implementation of supervision, providing access to training and development.
  • Ensure the practice environment is conducive to quality learning and assessment.
  • Identify and address training and educational needs of staff, maintaining your own continuing professional development.

Communication & Audit Responsibilities:

  • Communicate effectively with all staff members, respecting cultural and language differences.
  • Manage conflicts appropriately among staff, patients, and their families.
  • Ensure compliance with organisational and professional standards in both written and electronic communication.
  • Participate in relevant meetings related to the ANP role, including governance and departmental management.

Leadership & Managerial Responsibilities:

  • Foster a learning culture that supports staff through change and development.
  • Act as a professional role model, promoting high standards of practice and strong professional values.
  • Collaborate with the line manager to monitor expenditure and assist in budget management.
  • Develop evidence-based policies and procedures as required.

Key Criteria:

Essential:

  • Registered Nurse with 5 years of relevant community experience.
  • Degree in a related field.
  • Master’s in Advanced Practice (ANP) qualification.
  • Non-medical prescribing qualification.
  • Management qualification.
  • Excellent communication skills, particularly in multidisciplinary team working.
  • High professional standards.
  • Strong interpersonal skills.
  • Proven leadership abilities.
  • Ability to maintain confidentiality.
  • Enthusiastic and motivated.
  • Positive attitude.
  • Credible and trustworthy.

Desirable:

  • MSc in Management or an advanced clinical skills qualification.
