Job Title: Band 6 Occupational Therapist – Intermediate Care & Community (I&CC)
Location: Swindon, UK
Salary: £26 per hour (Hourly)
Duration: 3 months
A fantastic opportunity awaits! We are on the lookout for a Band 6 Occupational Therapist specialising in Intermediate Care & Community for a locum position in the vibrant area of Swindon. What you will do:
- Conduct comprehensive patient assessments to create personalized care plans that promote health and well-being within the community.
- Collaborate with multidisciplinary teams to facilitate seamless transitions and interventions that prevent hospital admissions and support effective discharges.
- Manage your caseload independently and ensure high-quality, patient-centred care.
- Implement evidence-based practice and treatment strategies, adjusting plans to meet evolving patient needs.
- Engage with families, carers, and community services to ensure that holistic support is in place for patients.
- Participate in training and development sessions to lead and support junior team members.
- Reporting to - Front Door Team Lead
- The staff member will be required to work their contracted hours over 7 days a week.
Requirements:
- State Registered HCPC.
- Diploma/Degree in Physiotherapy or Occupational Therapy
- Experience in complex discharge planning within the NHS
- Evidence of postgraduate learning relevant to clinical area
- Working as part of a MDT and joint problem solving.
- Supervision and training of others.
- Broad clinical experience e.g. surgical, ED, neurology, medical, orthopaedic, hand injuries, rehabilitation.
- Experience of providing a broad range of interventions for a complex caseload.
- Use of goal setting and outcome measures
- Experience of providing supervision for junior staff or assistants.
- Lead for student supervision.
- Carrying out and receiving appraisal.
- Experience in and an understanding of clinical governance e.g. audit, risk management, education.
