A fantastic opportunity awaits! We are on the lookout for a Band 6 Occupational Therapist specialising in Intermediate Care & Community for a locum position in the vibrant area of Swindon. What you will do:

  • Conduct comprehensive patient assessments to create personalized care plans that promote health and well-being within the community.
  • Collaborate with multidisciplinary teams to facilitate seamless transitions and interventions that prevent hospital admissions and support effective discharges.
  • Manage your caseload independently and ensure high-quality, patient-centred care.
  • Implement evidence-based practice and treatment strategies, adjusting plans to meet evolving patient needs.
  • Engage with families, carers, and community services to ensure that holistic support is in place for patients.
  • Participate in training and development sessions to lead and support junior team members.
  • Reporting to - Front Door Team Lead
  • The staff member will be required to work their contracted hours over 7 days a week.

Requirements:

  • State Registered HCPC.
  • Diploma/Degree in Physiotherapy or Occupational Therapy
  • Experience in complex discharge planning within the NHS
  • Evidence of postgraduate learning relevant to clinical area
  • Working as part of a MDT and joint problem solving.
  • Supervision and training of others.
  • Broad clinical experience e.g. surgical, ED, neurology, medical, orthopaedic, hand injuries, rehabilitation.
  • Experience of providing a broad range of interventions for a complex caseload.
  • Use of goal setting and outcome measures
  • Experience of providing supervision for junior staff or assistants.
  • Lead for student supervision.
  • Carrying out and receiving appraisal.
  • Experience in and an understanding of clinical governance e.g. audit, risk management, education.
