What you will do:

- Conduct comprehensive occupational therapy assessments to identify individual patient needs and develop tailored treatment plans.
- Facilitate safe and effective discharges, ensuring patients are well-prepared to manage their daily activities independently.
- Arrange for the provision of appropriate equipment to enhance patient independence and safety in their homes.
- Utilize recognized outcome measures to monitor patient progress and adjust treatment plans as necessary.
- Conduct family meetings to involve and educate family members about the patient's care plan.
- Collaborate with other healthcare professionals to deliver integrated care and maintain clear communication across the multidisciplinary team.

Requirements:

- Post-qualification experience as an Occupational Therapist, ideally in a rehabilitation setting.
- Must be registered with the Health and Care Professions Council (HCPC).
- A valid driver's license is required to perform this role effectively.

- Strong clinical reasoning and assessment skills.
- Excellent communication and interpersonal skills, vital for working autonomously and as part of a multidisciplinary team.
- Experience in using recognized outcome measures.
- Strong organisational and time management skills.
