Job Title: Band 6 Frailty Occupational Therapist

Join our team as a Band 6 Frailty Occupational Therapist with the Frailty Team in Bangor, UK. This ongoing full-time position provides a fantastic opportunity to contribute to patient care within the NHS. We are seeking a skilled professional with previous NHS experience and HCPC registration for a rewarding and challenging role.
Key Responsibilities:
- Comprehensive Assessments: Conduct thorough assessments of patients to determine their therapy needs and develop personalised treatment plans to promote recovery and independence.
- Multidisciplinary Collaboration: Work closely with doctors, nurses, social workers, and other healthcare professionals to deliver holistic, patient-centred care.
- Therapy Sessions & Interventions: Implement tailored therapy sessions and interventions designed to improve patients' independence, mobility, and overall quality of life.
- Patient and Family Education: Provide guidance and support to patients and their families, helping them manage frailty and adapt to home care.
- Documentation & Record-Keeping: Maintain accurate, up-to-date records of patient progress, care plans, and interventions to ensure high-quality service delivery.
- Community Visits: Travel to various patient locations as required, ensuring the delivery of care in community settings. A valid driving licence is essential for this role.
