Role Purpose:
As an Assistant Care Manager, you will work as part of a multi-disciplinary team to assess and support individuals with learning disabilities, ensuring they receive high-quality care and services. You will be responsible for carrying out assessments to identify Care Act eligible needs and ensuring compliance with key legislative frameworks, such as the Mental Capacity Act 2005 and the Care Act 2014.

You will support service users in promoting their independence by creating effective care plans, providing guidance, and signposting to additional services. You will also play a key role in coordinating services, arranging provision of equipment or minor adaptations, and working closely with health, education, and social care professionals to deliver integrated service delivery.

Key Responsibilities:

  • Work within and demonstrate compliance with appropriate legislative frameworks.
  • Participate as a member of a multi-disciplinary team, conducting assessments to identify Care Act eligible needs.
  • Ensure that the principles of the Mental Capacity Act 2005 and Care Act 2014 underpin practice.
  • Use the care-planning process to identify the most appropriate ways to promote independence for service users.
  • Provide advice, information, and signpost to other agencies and support services when necessary.
  • Arrange for the provision of services, equipment, and minor adaptations as required.
  • Collaborate with social care, health, and education professionals, as well as community resources, to deliver integrated care.

Requirements of the Assistant Care Manager:

  • NVQ Level 3 in Care.
  • Experience and understanding of working in either a health or social care environment.
  • Knowledge of the needs of older people, disabled individuals, and their carers.
  • A thorough understanding of the role and needs of informal carers.
  • Experience and expertise in care management processes.
  • Ability to undertake assessments, identify needs/risks, and produce care plans.
  • Understanding of the dynamics and challenges of working within a multi-disciplinary team.
  • Ability to work independently and follow direction when necessary.
  • Demonstrates accountability and responsibility in the role, using formal and informal supervision as required.
  • Excellent communication skills, both oral and written, to engage effectively with service users, carers, colleagues, and external agencies.
  • A good understanding of discrimination and inequality issues.
  • Competence in using Information Technology and being computer literate.
