Key Responsibilities:
- Conducting assessments of service users' needs and creating personalised care plans.
- Providing support and guidance to service users, their families, and caregivers.
- Collaborating with other professionals and agencies to ensure holistic support for service users.
- Reviewing and updating care plans regularly to reflect changing needs and circumstances.
- Advocating for service users to ensure their rights and preferences are respected.
- Working within legal and regulatory frameworks to safeguard vulnerable adults.
- Maintaining accurate records and documentation of interactions and interventions.
Requirements:
- Qualified Social Worker with a relevant degree and Social Work England registration.
- Proven experience in adult social work, preferably within a similar setting.
- Strong assessment and analytical skills, with the ability to identify needs and risks effectively.
- Excellent communication and interpersonal skills, with the ability to build rapport and trust.
- Ability to work effectively within a multidisciplinary team and collaborate with external partners.
- Sound knowledge of relevant legislation, policies, and procedures related to adult social care.
- Commitment to continuous professional development and adherence to ethical standards.
