Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for a Social Worker within the Reviewing Team to work full time based in Bury, Greater Manchester.

Location: Bury, Greater Manchester

Working Hours: Full-time, 37 hours per week

Contract: Permanent

Salary: £29,269 per annum

Main Duties as a Social Worker:

You will play a vital role in supporting customers and carers by reviewing and enhancing individual support plans. Your responsibilities will include providing valuable advice, guiding clients to appropriate resources, and ensuring high standards of practice are maintained.

  • Support Plan Management: Collaborate with customers and carers to review and update individual support plans, adapting them to meet evolving needs. Ensure that all changes follow internal procedures and are implemented promptly.
  • Resource Advice and Information: Offer expert advice and information about a range of community-based resources, including those from voluntary, private, and independent sectors. Help customers and carers access universal and preventative services effectively.
  • Promoting Independence: Empower customers and carers to live independently by supporting them in making informed choices. Utilise their existing strengths and support networks to enhance their self-sufficiency.
  • Signposting Services: Direct customers and carers to suitable community services that address their needs and support their goals. Facilitate connections with services that will help achieve identified outcomes.
  • Resource Knowledge: Stay updated on available resources such as assistive technology, equipment, and community-based services. Use this knowledge to support the review and development of support plans.
  • Legislation and Best Practice: Maintain current knowledge of relevant legislation and best practices in social care. Apply this knowledge to ensure compliance and enhance the effectiveness of the review process.
  • Budget Support: Assist customers and carers in achieving their desired outcomes using their personal budget. Provide guidance on how to effectively utilize available funds to meet their needs.
  • Performance and Data Management: Contribute to the development of the Reviewing Team's performance framework. Ensure accurate data capture, adherence to best practices, and identification of barriers to performance improvement.
  • Subject Expertise: Take on the role of subject lead when required, staying informed about specific areas of expertise. Provide updates and support to team members on changes or emerging best practices.
  • Team Collaboration: Work collaboratively within a multi-disciplinary team, participating in meetings and contributing to team objectives. Support colleagues in enhancing customer and carer support plans through quality improvement efforts.
  • Record Keeping: Maintain accurate and up-to-date service user records in line with departmental policies and requirements. Ensure documentation reflects the support provided and any changes made.
  • Workload Management: Effectively manage and prioritize your workload, including home visits across the Borough. Demonstrate strong time management and planning skills to ensure efficiency.
  • Supervision and Development: Actively engage in supervision sessions and performance reviews. Contribute to identifying your training and development needs to enhance your professional growth.

Key Requirements for this role:

  • Must possess a recognised social work qualification
  • Registered with Social Work England
  • Previous experience in assessing customer needs and developing individual support plans. Demonstrated ability to support vulnerable adults and/or their carers effectively.
  • Effective verbal and written communication skills, able to convey complex information clearly to diverse audiences.
  • Proficient in liaising with families, carers, and multidisciplinary teams.
  • Experience working collaboratively within a multi-disciplinary team, contributing to collective goals and integrating diverse perspectives.
  • Competence in maintaining accurate records and using IT systems.
