Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for a Adult Social Worker within the Long Term Care Team to work full time based in Vale of Glamorgan.
The salary for this permanent Social Worker job is up to £41,496 per annum.
Main duties:
- To undertake assessment of citizens in accordance with statutory duties under relevant legislation.
- To plan commission services to enable adults with care & support needs and carers with support needs to overcome the barriers to achieving the personal outcomes and monitor and review the care plans through co-production with citizens and carers.
- To work in partnership with citizens with care and support needs, carers with support needs and adults at risk to provide the best opportunities for people to live fulfilled lives in a way that matters to them.
- To negotiate and co-ordinate applications for resources and to work in partnership with internal and external partners both statutory and voluntary on behalf of citizens and carers.
- To undertake case management responsibility for a specific caseload and prioritise workload demands appropriately.
- To maintain up to date electronic, case files and relevant documents, providing accurate records relating to performance management and other statistical data as required.
- To take part in office duty rotas and other activities associated with a multi-disciplinary team.
- Ensure compliance with the Councils Financial Regulations, Standing Orders, Policies and Procedures.
- To adhere to Heath & Safety legislation/relevant Council policies and procedures and to take reasonable care for the health and safety of yourself and other persons who may be adversely affected by your acts/omissions.
- Such other duties and responsibilities commensurate with the grade and in accordance with the general character of the job as may reasonably be required by the Chief Officer from time to time.
Requirements of this Social Worker job:
- A social Work Qualification or equivalent.
- Understanding of relevant legislation.
- Continual professional development.
- Social Work England registered.
