About the Role:
As a Specialist Social Worker in our High Needs Team, you will be responsible for providing comprehensive support and assistance to individuals with mental health challenges and learning disabilities. You will assess their needs, develop tailored care plans, and work collaboratively with other professionals and agencies to ensure the delivery of high-quality services. Your role will involve direct client work, advocacy, and facilitating access to resources and support networks.

Key Responsibilities:
- Conducting assessments to identify the needs of individuals with mental health issues and learning disabilities.
- Developing and implementing person-centred care plans in collaboration with service users, their families, and multidisciplinary teams.
- Providing support and advocacy to empower individuals to access appropriate services and resources.
- Monitoring and reviewing the effectiveness of interventions and adjusting as necessary.
- Collaborating with external agencies and partners to coordinate holistic support for service users.
- Maintaining accurate and up-to-date records in accordance with legal and regulatory requirements.
Requirements:
- Qualified Social Worker with Social Work England registration.
- Experience working with individuals with mental health issues and learning disabilities.
- Strong assessment, intervention, and care planning skills.
- Excellent communication and interpersonal skills, with the ability to build rapport and establish trust with service users and their families.
- Ability to work effectively within a multidisciplinary team and collaborate with external stakeholders.
