Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for a Social Worker within the Integrated Neighbourhood Team to work full time based in Bury, Greater Manchester.

Location: Bury, Greater Manchester

Working Hours: Full-time, 37 hours per week

Contract: Permanent

Salary: £36,648 per annum

Main Duties as a Social Work Advanced Practitioner:

  • Assessment and Support Planning: Conduct comprehensive assessments of adults’ needs within the integrated neighbourhood. Develop, implement, and review personalised support plans to address those needs, ensuring alignment with the individual's goals and preferences.
  • Case Management: Manage a caseload of individuals, providing consistent support and monitoring their progress. Adjust support plans as needed based on changing circumstances and evolving needs.
  • Coordination of Services: Collaborate with other health and social care professionals, including medical teams, community organisations, and voluntary agencies, to coordinate and deliver integrated care services.
  • Community Resource Utilisation: Identify and connect individuals with appropriate community resources, including housing, financial support, and recreational activities. Provide information and referrals to help clients access these resources effectively.
  • Safeguarding: Identify and respond to safeguarding concerns in accordance with statutory guidelines and local policies. Report and manage cases of abuse or neglect to ensure the safety and well-being of vulnerable adults.
  • Documentation and Reporting: Maintain accurate and up-to-date records of assessments, support plans, and interactions. Prepare detailed reports for case reviews, meetings, and statutory requirements.

Key Requirements for this role:

  • Must possess a recognised social work qualification
  • Registered with Social Work England
  • Comprehensive understanding of current safeguarding policies and procedures to ensure their consistent and effective application.
  • Capable of actively contributing to the creation and implementation of team and service plans.
  • Ability to liaise effectively with individuals such as families, carers, and multidisciplinary team members, fostering collaboration and ensuring effective service delivery.
  • Understand the functionality of a duty system and related casework. Provide relevant advice and support to multidisciplinary colleagues by sharing your expertise and experience.
