Locum Opportunity: Social Worker - Integrated Independence Team in Hackney, UK - Earn £32 per hour! What you will do:
- Collaborate with individuals, carers, and professional staff across various agencies and service providers to develop care plans that promote independence, choice, and control.
- Manage a diverse caseload of individuals and carers requiring support, guidance, and close monitoring.
- Respond swiftly to service user needs while also offering short-term bed-based interventions where needed, ensuring care is delivered effectively at home or in familiar environments.
- Provide guidance and direction to colleagues, supporting senior team members and managers as necessary.
- Adhere to the standards of proficiency set by the Health and Care Professions Council, maintaining registration and focusing on continuous learning and development.
- Contribute to the overall performance and reputation of the Council through dedicated service delivery and engagement.
