About the Role:
- This role is based within the Richmond and Barnes Social Care Team, working alongside Social Care Assessors, District Nurses, and Community Matrons.
- The team provides comprehensive, strength-based services to adults aged 18 and over with physical disabilities, and older adults with physical and mental health conditions.
- The office is strategically located just 12 minutes from Clapham Junction in East Sheen, Wandsworth, and offers excellent local facilities.
What You Will Do:
- Conduct assessments of service users and carers to identify needs and risks, promoting independence, providing support, and preventing harm.
- Work collaboratively with service users and carers, using a person-centred approach to maximise independence, wellbeing, and choice.
- Apply social work principles and values, embracing diversity to guide professional practice.
- Develop care and support plans that deliver personalised, integrated care to help individuals maintain their independence and achieve their goals.
- Engage in safeguarding practices and receive specialist training and supervision to enhance this critical area of practice.
Essential Qualifications, Skills, and Experience:
- In-depth knowledge of Social Work Professional Standards, relevant legislation, and national guidelines in adult social care services.
- Extensive post-qualification experience in conducting social care assessments and developing care plans in a statutory social care or health setting.
- A strong understanding of the local authority’s statutory responsibility to safeguard vulnerable adults, with the ability to collaborate effectively with partners.
