What you will do:

- Conduct needs-based assessments and develop personalised care plans in line with the Care Act 2014.
- Manage safeguarding concerns, perform Section 42 Enquiries, and implement robust protection plans.
- Coordinate support by arranging and reviewing care packages, ensuring that service users have access to appropriate services tailored to their needs.
- Offer crisis management by providing timely interventions to prevent care breakdowns or unnecessary hospital admissions.
- Advocate for service users' rights and collaborate effectively with families, carers, and multi-agency teams to ensure comprehensive support.
- Ensure compliance with relevant legislation, policies, and best practice guidelines, maintaining high professional standards.
- Maintain accurate case records and prepare detailed formal reports to communicate effectively with relevant stakeholders.
- Engage in ongoing professional development to stay updated on changes in legislation and best practices.
