What You Will Do:
- Conduct Care Act (2014) assessments and reviews using both telephone and face-to-face methods.
- Manage a caseload efficiently, ensuring timely recording of interventions to support effective case management.
- Build and maintain strong, professional relationships with partner agencies, such as Health, Police, Housing, and the Private and Voluntary Sector (PVI).
- Identify opportunities to support carers, promote independence, and enhance the wellbeing of service users.
- Respond to urgent situations requiring duty cover, triaging and assessing incoming work for appropriate action.
- Lead Section 42 Safeguarding efforts, chairing multi-agency meetings, and conducting Mental Capacity Act (2005) assessments.
