What you will do:

  • Engage with the Locality Team to coordinate and review care management plans and support arrangements including extra care housing and direct payments.
  • Conduct assessments in line with the Care Act 2014 and assist in transitioning young people from Children’s services to Adult services.
  • Support younger adults with physical and sensory impairments, as well as older individuals, to maintain independence, working closely with their carers.
  • Collaborate with partner agencies to develop strong working relationships, ensuring comprehensive care.
  • Act as a duty worker on a rota basis, facilitate hospital discharges, and manage safeguarding enquiries.
  • Perform Mental Capacity Assessments and Best Interest decisions, maintaining the high standards expected within the team.
