The salary for this permanent Rotational Clinical Pharmacist job is £35,392 - £42,618 per annum.
Main Duties:
- Deliver high-quality patient-centred care in line with Clinical Governance principles.
- Ensure compliance with Medicines Management, Controls Assurance Policies, and Treatment Protocols.
- Investigate patients' drug histories when necessary, including assessing patients' own medications (POD) for potential reuse.
- Review prescriptions for legality, clarity, clinical accuracy, and suitability for the patient.
- Monitor drug usage in patients, reporting near misses in prescribing and administration, as well as addressing risk management issues.
- Coordinate the timely supply of medications to patients in collaboration with nurses, medical staff, ward Pharmacy Technicians, and Dispensary staff.
- Foster strong working relationships with service users.
- Devise and monitor medication storage on wards to ensure the maintenance of potency, security, and quality.
- Provide medicines information to prescribers, nurses, midwives, healthcare professionals, patients, and carers.
- Offer advice to patients and carers to enhance adherence to medication regimens and assess the need for compliance aids.
- Manage and prioritise workloads, including those of other Pharmacy staff, to meet patient and service requirements.
- Participate in rotational training programmes to gain broad experience in Clinical Pharmacy.
- Provide coverage for other ward pharmacists as needed.
- Act as the supervising pharmacist on rotation in the dispensary, conducting clinical checks of prescriptions to ensure legality, accuracy, appropriateness, and policy compliance.
- Prioritise and clinically check prescriptions in the appropriate order.
- Perform final checks on dispensed medicines as required.
- Provide advice to patients and carers to improve medication adherence.
- Conduct checks on formulae and worksheets for extemporaneous aseptic preparations, such as cytotoxics, parenteral nutrition, and non-aseptic preparations.
- Carry out final checks on ingredients, batch numbers, and expiry dates for extemporaneous dispensing when needed.
- Respond to medicine information queries as appropriate.
- Dispense prescriptions and manage the supply of stock medicines and Controlled Drugs as required.
- Handle prescription payments efficiently.
- Ensure compliance with health and safety regulations and other relevant legislation governing the safe and secure handling of medicines.
- Provide coverage for pharmacists at other sites when necessary.
Requirements of the Rotational Clinical Pharmacist:
- Pharmacy degree
- Completion of 1-year pre-registration training
- Registered member of the General Pharmaceutical Council (GPhC)
- Dispensing and checking skills
- Clinical pharmacy experience within a hospital setting
- Experience in providing medicines information
- Ability to travel between sites and for On-call duties
