Pay Rate: £26 - £28 per hour (assignment rate)

Duties of the Occupational Therapist:
- To work as a clinically independent and autonomous practitioner , participate and manage a defined caseload as an Older People’s AHP Practitioner in ED/CDU/Same Day Emergency Care Frailty beds, OPRAC clinic.
- Carries out patient assessment ,implements and evaluates best course of action through the following activities: physical examination, health assessment and diagnosis, therapeutic consultation skills ,requesting, monitoring and analysing appropriate diagnostic and or therapeutic investigation/treatment, care and pharmacology.
- To provide expert clinical practitioner assessment and plan ,implement and evaluate comprehensive programs of care ,providing specialist advice and maintaining associated records. Utilizing AHP Practitioner knowledge and skills for high levels of decision making , expert problem solving abilities and disease processes.
- To work with multidisciplinary team and liase and refer on as necessary to appropriate health care professionals within primary and secondary care.
- To act as a source of expertise and knowledge for Elderly patients ,their carers and their families within LNWH NHS Trust, within the remit of Frailty Practitioner.
- Undertake Comprehensive Geriatric assessment in line with the British Geriatric Society Standards
- To facilitate the development of Older peoples AHP skills and knowledge for own CPD and for other members of the Ambulatory Care and Emergency Departments and Department of Medicine and to meet other specific education and developmental needs, to meet Frailty service needs.
Requirements of the Occupational Therapist:
- Degree or equivalent in Occupational Therapy
- Recent UK experience working as an Occupational Therapist
- Recent NHS experience
- Registered with the HCPC
