What you will do:

  • Carry out needs-led assessments of occupational performance for clients with a variety of medical conditions and physical disabilities, creating individualised action plans.
  • Collaborate with clients and carers to recommend suitable equipment, adaptations or re-housing, considering all factors including legal and resource implications.
  • Act as a locality lead to devise and implement innovative, integrated housing solutions for adults with complex needs, extending beyond traditional services.
  • Conduct risk assessments and advise on safe independence strategies within the home environment, while coordinating with care management teams to adjust care packages as needed.
  • Exercise autonomy in managing a complex clinical caseload, providing expertise and guidance to fellow Occupational Therapists and Assessment Officers on challenging cases.
  • Offer specialist advice and support on complex manual handling situations, working jointly and providing training as necessary.
  • Engage in referral triaging to allocate resources efficiently, supporting the Adult Contact Team in assessing needs and offering self-provision advice where required.
  • Promote and encourage the use of assistive technology, such as Telecare, to enhance client independence.
  • Facilitate accessible housing management, aligning client needs with available adaptations and completing necessary property suitability checks.
  • Maintain thorough and precise records conforming to local policy, contributing data for Performance Indicators and ensuring compliance across the team.
  • Participate in ongoing professional development through the appraisal process and support student training within the occupational therapy field.


Requirements:
You must be HCPC registered with a minimum of 2 years' experience in the field. A car driver is essential as you will operate across multiple sites.
