What you will do:
- Lead assessments and create personalised care plans for individuals with complex mental health issues, ensuring pathways to support recovery and well-being.
- Work collaboratively with a multidisciplinary team to deliver comprehensive mental health services.
- Engage in risk assessment and management, considering the wider impacts of health and social needs.
- Manage a diverse caseload of service users, offering both base and community-based support, making a driving licence essential.

Requirements:
- Must have HCPC registration.
- A minimum of two years of experience in a similar role is essential.
- Proven experience in working clinically with service users experiencing a range of mental health problems.
- Ability to assess, plan, implement and review care needs alongside a multidisciplinary team.
- Strong understanding and experience in risk assessment and management.
